- Before applying to become a medical patient, make sure you meet the following qualifications:

18 years of age or older, or a pediatric patient with parental consent

You have a Physician Certification from an MD or DO licensed in Delaware (Read “Talking With Your Doctor”) OR be 65 years or older to self ceritfy.

A Delaware resident with proof of residency (DE Driver’s License or DE State ID)

Become a medical caregiver
If you’re homebound or need assistance, you can designate a caregiver to purchase the medication on your behalf. Caregivers must be approved by the State through an application process.
